Effective policy that is not good policy I’ve had a rather interesting debate the other day. It started from a tweet, then took a turn into discussion in a online forum, and eventually a blog post. It is OK to treat Maori like a special and very different minority group, because that’s just how the world works. If someone is a Maori, they are better than someone who is not, and has a special moral entitlement to being treated as such. That this is the right approach to policy-making should be obvious. In fact, it is more an indictment of a particular view of politics than the actual policy that is under discussion. ( I really ought to do a bit more deconstruction of Labour politics. To describe radiographic documentation of otitis media (OM)-related symptoms and infections in children aged 2 to 60 months who presented to a pediatric emergency department (PED) in an urban community hospital in North Texas. This retrospective chart review involved a 5% random sample of patients that presented to the PED between January 1, 2002 and December 31, 2003. Patients were included if they presented with a chief complaint of otitis media or a chief complaint of fever and had had an radiograph interpreted as “possible acute OM.” The indications for and the quality of the radiographs were evaluated. Two hundred three radiographs, 71% of which were requested by nonradiologists, were reviewed. In the group of children who presented with a chief complaint of fever alone, 11 (14%) of the radiographs showed OM. In the group of children who presented with a chief complaint of fever with an additional chief complaint of OM, 8 (80%) of the radiographs showed OM. In the group of children who presented with a chief complaint of fever and a history of at least three of the four symptoms of OM, 23 (56%) of the radiographs showed OM. The radiographs were of excellent or adequate quality in 63% of the cases. Improved physician education and supervision is needed to increase the use of radiographic examinations in the evaluation of children with suspected OM.
