Although the mechanisms and risk factors for poor survival after stroke have been identified, the effect of brain structural damage on acute mortality and survival of stroke patients remains unclear. Therefore, the aim of this study was to investigate the effect of brain structural damage on acute mortality and 1-year survival of patients with stroke. We conducted a cohort study of consecutive stroke patients admitted to inpatient neurological wards in six tertiary hospitals in Japan from January 2005 to March 2006. Acute mortality and 1-year survival rates were compared between the subjects with no structural brain damage (N-BSD) and those with structural brain damage (S-BSD) using logistic regression and Cox proportional hazards models, respectively. Of 2996 stroke patients enrolled, 1236 (41.4%) and 851 (29.2%) had N-BSD and S-BSD, respectively. S-BSD was independently associated with acute mortality (odds ratio, 1.72; 95% confidence interval, 1.37-2.16) and 1-year mortality (hazard ratio, 1.55; 95% confidence interval, 1.30-1.86) after adjusting for age, gender, diabetes mellitus, hyperlipidemia, atrial fibrillation, coronary artery disease, and stroke severity (National Institute of Health Stroke Scale score).
